Loot Chests
989 chests scattered across the world. Take one item and the chest locks for twenty minutes.
There are 989 loot chests hidden across the world, in every region from the coast to the mountains. They are ordinary-looking chests, and they hold real items in a real inventory.
The catch is the lock.
One chest, one opening
A loot chest is not a chest you loot. It is a chest you race for.
The moment the first item leaves the chest, it locks. Nobody else can open it until its timer runs out - and the timer is per chest, not per player. There is one inventory and one lock state, so two players cannot be on separate timers for the same chest.
When the timer expires, the next person to open it finds a completely fresh chest, rolled at that moment. Between 20 and 30 minutes, decided per chest.
Because the lock is on the chest and not on you, arriving second is worthless. Either you get there first, or you keep walking - and coming back later means coming back to something someone else may already have taken.
What is inside
Each chest is rolled from its region's table, which gives 2 to 5 stacks of goods plus 1 to 3 combat rolls. Higher-tier regions roll more.
The chests hold combat gear only - swords, armour, arrows, food, potions, golden apples, ender pearls, totems. The random economy filler that the system supports is switched off, so a chest is never a pile of dirt and dye. Every chest is worth the trip.
What you actually pull is weighted by value: the cheapest thing in a tier is the most common, the dearest is the rarest. A chest gives mostly ordinary goods with an occasional good pull.
The four tiers
Chests are tiered by the region they sit in, and the tiers track the level bands - so the loot in a chest is roughly what someone who can survive there should be carrying.

Loot is level-gated too
A chest can only drop gear the region is allowed to hand out. Each region uses the bottom of its level band as its gate, so an iron sword (level 18) never appears in a Lv 1-5 zone no matter how lucky the roll.

This is the same ladder equipment requirements use, so a chest can never hand you something you are not yet allowed to hold.
Safe zones
Within 128 blocks of spawn, nothing hostile spawns and no spawners exist. The chests are still there - it is only the mobs that are not.
